A small number of cells resembling true apocrine cells are found in the breast, and they may develop carcinoma. (a) On H&E staining, apocrine cells are characterised by abundant pink, granular cytoplasm and centrally located hyperchromatic nuclei, which may have one or two prominent nucleoli. The nuclear to cytoplasmic ratio is very low. The apical portions of the cells contain coarse eosinophilic granules. Occasionally, apocrine cells may be atypical and present diagnostic difficulties.
Carcinoma with apocrine differentiation (invasive apocrine carcinoma) is defined by “WHO Classification of Tumours of the Breast” as “any invasive carcinoma in which cells show the cytological features of apocrine cells” (O’Malley and Lakhani 2012).
